This questionnaire is designed to gather information and opinions on the use of evidence-based practice amongst health professionals. There are no right or wrong answers for we are interested in your opinions and your own use of evidence in your practice.  

By completing the following evaluation questionnaire, your journal club will receive feedback on the effects of your journal club to assist with future continuous improvement for your service. You will also have access to additional journal club resources on this website. The evaluation will take approximately 5 minutes before your participation and 15 minutes after your participation. Completion of this evaluation implies that you agree for your de-identified responses to be used as part of an international research study aiming to help health professionals to use journal clubs and evidence-based practice to improve patient outcomes.
